Your provider will verify your eligibility and determine the appropriate dose based on your treatment history. To qualify for 4.4mg/week, you must have been prescribed at least 2.2mg/week within the past 45 days. To qualify for 7mg/week, you must have been prescribed at least 4.4mg/week within the past 45 days. To qualify for 10mg/week, you must have been prescribed at least 7mg/week within the past 45 days. Patients requesting 13mg/week must have documentation of prior treatment at an appropriate dose, as determined by the prescribing provider
